How to Apply to St. Alphonsus Parish School for 2012-2013 School Year
We request that all parents/guardians attend one of our Open House Events or schedule an individual tour before applying.
Application materials for the 2012-2013 academic year are due by Tuesday, February 28, 2012.
A completed application includes the following:
All
o Recent photo of your student to help us to identify the applicant
Preschool and Kindergarten
o $25 per student application fee
o Preschool or K-4 application form - open here: Preschool, K-4
First and Second Grade
o $25 per student application fee
o K-4 application form - open here: K-4
o Copies of most recent report card and standardized test scores (if available) from current school
Third and Fourth Grade
o $25 per student application fee

o Confidential Teacher Evaluation - available here
o Copies of most recent report card and standardized test scores (if available) from current school
Fifth through Eighth Grade
o $50 per student application fee

o Schedule parent/guardian meeting with Principal Maureen Reid*
o Schedule full-day student visit*
